Patient forms and facility inquiries

Prescription transfers and new-patient registration open CK’s existing provider forms. Facility inquiries open CK’s hosted Wix form and notify our LTC team by email. General facility inquiries must not include patient or prescription information. For prescription-specific questions, contact your pharmacy.

Welcome-message preferences

This website remembers which seasonal welcome message you have seen in your browser, so it does not repeatedly interrupt your visits. This preference contains no patient details. Clearing browser data resets it.

External patient services

Links to RxLocal, 123FormBuilder patient forms, CK’s hosted Wix inquiry form, and the nursing home staff portal lead to separately managed services. Information entered on those services is handled by the relevant service and the pharmacy. Please review the notices on each service before submitting information.

Website activity

We use general website activity to understand which services and links are useful. Our button-click events contain the type of action and, where relevant, the pharmacy location. They do not include patient names, prescription details, or form contents.
